We are advanced professional members of the Aging Life Care Association™, licensed independent clinical social workers, and certified advanced social work case managers. We specialize in serving older adults including those approaching end of life. ​
What We Do
We conduct in-home assessments, develop personalized care plans, provide guidance and advocacy, and offer emotional support to individuals, couples, and families navigating the challenges of aging and/or living with a chronic medical condition.

We are trained to work in both the health care and mental health care systems. Social work brings a holistic and integrative view into the practice of caring for seniors. We have special expertise in the following 8 knowledge areas: health and disability, financial, housing, families, local resources, advocacy, legal, and crisis intervention.
